40 year old female with flank pain, burning micturition, recurrent fever associated with chills

Chief complaints
A 40 year old female, housewife, resident of Hyderabad presented to the OPD with chief complaints of
Pain in abdomen since 15 days
Burning micturition since 5-6 days
Fever associated with chills since 15 days
Frothy urine since 20 days
Pedal edema since 8-9 months
Vomitings since 15 days
History of Present illness
The patient was apparently asymptomatic 15 days back. Then she developed pain abdomen radiating from umbilicus to left lumbar region. She had vomitings& fever associated with chills. After 10 days patient had developed burning micturition and frothy urine.
Past history
Diabetic since 10 years on medication
Hypothyroidism since 8 years on medication
Hypertension since 11 years on medication
No CAD
No Asthma
No Epilepsy
No TB

Normal appetite
Adequate sleep
Mixed diet
Constipation since 2-3 days
Burning micturition

The patient is conscious, coherent, cooperative and well oriented

No icterus
No cyanosis
No clubbing of fingers
No lymphadenopathy
Pedal edema- Yes(pitting type)

Temperature- Afebrile
Pulse rate- 84 beats/min
Respiration rate- 16 cycles/min
Blood Pressure- 100/80 mm Hg
SPO2- 92%
